This Participation Agreement (“Agreement”) governs your child’s participation in the Teen-X program (the “Program”), produced and operated by Relief Alliance, a 501(c)(3) nonprofit organization, in partnership with Better Vision for Children. By signing below, you (“Parent/Guardian”) consent to the participation of your minor child (“Participant”) in the Program, including all associated media activities, interviews, recordings, off-camera engagements, and mentoring sessions. This Agreement constitutes a binding legal contract.
Participation in the Program is open only to minors between the ages of 10 and 19 who have the express written consent of a parent or legal guardian. By submitting the application form, you affirm that:
You acknowledge that your child’s participation in the Program may involve exposure to public environments, television production sets, travel, interviews with public figures, and other media-related activities. By consenting, you and your child voluntarily assume all such risks, including but not limited to:
To the fullest extent permitted by law, you agree to release, waive, indemnify, and hold harmless Relief Alliance, Better Vision for Children, their officers, employees, agents, volunteers, contractors, licensees, media partners, sponsors, and affiliates from any and all claims, liabilities, demands, actions, suits, costs, expenses, or damages arising out of or relating to your child’s participation in the Program—including but not limited to claims of negligence, defamation, invasion of privacy, misappropriation of likeness, or injury.
You grant full, unrestricted, royalty-free, irrevocable permission for the Program and its affiliates to:
Submission of an application does not guarantee acceptance into the Program or any particular level of visibility or airtime. Program selections are made at the sole discretion of Relief Alliance and its production team. No promises or representations are made regarding talent development, exposure, or future opportunities.
Participants must adhere to all behavioral guidelines set by the Program. Relief Alliance reserves the right to remove a Participant from the Program at any time for conduct deemed inappropriate, unsafe, disrespectful, or otherwise disruptive to the mission and integrity of the Program. Dismissal decisions are final and not subject to appeal.
You and your child agree not to disclose or disseminate confidential production details, including but not limited to scripts, unreleased footage, internal communications, or identities of other participants, without prior written permission.
This Agreement shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of laws principles. Any disputes shall be resolved exclusively through binding arbitration in Osceola County, Florida, under the rules of the American Arbitration Association. You waive the right to participate in a class action or jury trial.
If any part of this Agreement is deemed unenforceable by a court of competent jurisdiction, the remainder shall remain in full force and effect.
This document represents the entire understanding between the parties regarding the subject matter herein and supersedes all prior oral or written representations.
By signing below, you acknowledge that you have read, understood, and agreed to all terms set forth in this Agreement. You further confirm that you are the legal parent or guardian of the named minor participant.

This Agreement (“Agreement”) is entered into by and between Relief Alliance, a registered 501(c)(3) nonprofit corporation (“RA”), and the undersigned minor individual (“Minor Participant”) and their parent or legal guardian (“Guardian,” and collectively with the Minor Participant, the “Participant”), for participation in the Teen-X TV Program (“Teen-X” or the “Program”).
This Agreement supersedes any and all prior, concurrent, or future oral or written agreements between the parties regarding Teen-X or any rebranded successor program, and shall remain fully enforceable in any form, platform, or naming iteration thereof.
This is a web-based agreement maintained by Relief Alliance and may be updated at any time to reflect legal, operational, or organizational changes. Relief Alliance reserves the right to modify the terms and conditions of this agreement at its sole discretion and without prior notice.
The most current version of this agreement will always be accessible on the Relief Alliance website. It is the responsibility of the participant and/or their parent or legal guardian to review and accept the most up-to-date version prior to participation in any Relief Alliance-sponsored event or activity.
By submitting this form electronically on the designated web page, the participant and their parent or legal guardian acknowledge that:
1.1 Weekly Programming and Content Production
Minor Participant shall serve as a principal content contributor to Teen-X, appearing in and participating in weekly one-hour digital sessions conducted via StreamYard or such other platform designated by RA. These sessions may include but are not limited to: panel discussions, peer interviews, guest interviews, and editorial commentary.
1.2 Field Coverage and Equipment Use
The Minor Participant shall also be responsible for covering approved events in person as a field correspondent representing Teen-X. This includes operating camera and audio recording equipment to capture interviews, event footage, and narrative content, subject to RA’s editorial direction.
1.3 Guardian Meeting Participation
While only the Minor Participant is authorized to appear on the Program or contribute to its editorial content, Guardians may attend internal planning or administrative meetings upon request or approval by RA.
2.1 Dues and Financial Contributions
Participant acknowledges that dues may be assessed by RA in support of Teen-X’s operations, including equipment acquisition, software licensing, platform hosting, and other production costs. Dues are assessed at the sole discretion of RA and are subject to change with written notice.
Participation in the Program is contingent upon timely and full payment of all dues. Failure to pay dues in full will result in the immediate suspension of the Participant’s right to contribute to or appear on Teen-X until such obligations are satisfied.
2.2 Equipment Ownership and Use
Any equipment—including but not limited to cameras, microphones, lighting, tripods, or accessories—issued to the Minor Participant for use in Teen-X is and shall remain the exclusive property of Teen-X, and by extension, Relief Alliance. No title or ownership interest is transferred to the Participant or Guardian under this Agreement.
Equipment must be used solely for Teen-X purposes, maintained in good working condition, and returned promptly upon request by RA. Any damage, loss, theft, or missing components shall be the sole financial responsibility of the Guardian, who agrees to either repair, replace, or reimburse RA for the fair market value of the item(s), at RA’s sole discretion.
Upon termination of this Agreement or the conclusion of the Minor Participant’s involvement, all equipment must be returned to RA in its original condition (ordinary wear and tear excepted), where it shall be reallocated for continued Teen-X use.
3.1 Grant of Rights
Participant irrevocably grants RA the unlimited, worldwide, perpetual right to record, use, reproduce, edit, adapt, stream, display, and publish the Minor Participant’s image, voice, name, likeness, performance, personal statements, creative work, and submitted content (“Materials”) in any form and for any lawful purpose, including promotional, educational, and commercial use.
3.2 Ownership and Compensation
All rights to the Materials and derivative works are vested exclusively in RA. Participant waives all rights to royalties, residuals, or future compensation of any kind related to use of the Materials.
4.1 Definition of Confidential Information
Confidential Information includes, but is not limited to: internal communications, creative development, production plans, partner or sponsor relationships, strategic initiatives, and any non-public operational details related to Teen-X or RA.
4.2 Scope and Duration
Participant agrees not to disclose, reproduce, or use Confidential Information for any purpose outside of Teen-X without prior written authorization. This obligation applies retroactively and shall remain in force for five (5) years following the end of the Participant’s involvement in Teen-X.
5.1 Non-Solicitation
Participant shall not directly or indirectly solicit, engage, or attempt to work with any individual, organization, or contact introduced through their participation in Teen-X for purposes outside the Program, without the prior written consent of RA.
5.2 Derivative Work Prohibition
Participant agrees not to develop, sponsor, participate in, or launch any derivative work, show, podcast, or media initiative that replicates, borrows from, or is inspired by Teen-X’s format, content, branding, or structure, whether during or after their participation.
5.3 Duration
These non-circumvention obligations shall remain in effect for a period of five (5) years following termination of this Agreement.
6.1 Termination Rights
This Agreement may be terminated by either party, for any reason or no reason, upon thirty (30) days’ written notice. RA also reserves the right to terminate participation immediately in cases of material breach, misconduct, failure to pay dues, equipment misuse, or violation of RA’s values.
6.2 Post-Termination Obligations
All dues owed must be paid in full prior to final termination. All RA-issued equipment must be returned immediately upon request or conclusion of participation. Participant remains bound by all confidentiality, IP, and non-circumvention obligations following termination.
7.1 General Waiver
Participant and Guardian agree to hold harmless and release RA, its board members, employees, volunteers, representatives, and affiliates from any and all claims, demands, or causes of action arising from participation in Teen-X, including but not limited to personal injury, reputational harm, or property loss.
7.2 Third-Party Behavior
RA shall not be held responsible for unauthorized use or distribution of Participant content or image by third parties unaffiliated with RA.
8.1 Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State of Florida, without regard to conflict of law rules.
8.2 Severability
If any provision of this Agreement is found to be invalid or unenforceable, all remaining provisions shall remain in full force and effect.
8.3 Entire Agreement
This Agreement constitutes the entire understanding between the parties and supersedes any prior discussions, correspondence, or agreements. No modification shall be valid unless in writing and signed by all parties.
